St. Mel School has the following admission guidelines. St. Mel School is an integral part of the parish program that provides an extraordinary opportunity for the education of the whole child. All other things being equal, faithful parishioners are given priority and preferred consideration for acceptance into St. Mel School. Non-parishioners are accepted if space is available at a higher tuition level.
REQUIREMENTS FOR REGISTRATION:
Age: For Kindergarten, child should be five years of age on or before September 1, 2021.
For Grade 1 applicant, child must be six years of age on or before September 1, 2021.
Fee: $100.00 per student application (non-refundable)
Application: A completed application form must be returned to the school office as soon as possible but no later than Thursday, February 12, 2021. No application will be processed until all the following documents are presented to the school office.
For verification purposes only, we are required to inspect the following original documents. After verification, documents will be returned.
Original Birth Certificate – Document must have a State or County seal. No hospital copies will be accepted. You may obtain an original birth certificate by contacting the County Registrar of Records. For those born in Los Angeles County, contact the Van Nuys County Registrar of Records at: 14340 Sylvan (between Victory & Oxnard), Van Nuys, California, (818) 374-7176. Allow at least two weeks for delivery.
Original Baptismal Certificate – You may obtain an original Baptismal Certificate from your Church of Baptism. Allow at least two weeks for delivery.
Teacher Evaluation Report – This form must be completed by your child’s current teacher.
Assessment: A Readiness Assessment will be scheduled for each child after the aforementioned documents and fees are delivered to the school office. Assessments will be scheduled when it is deemed safe to do so.
Original First Reconciliation Certificate (for grades 3-8)
Original First Eucharist Certificate (for grades 3-8)
Photo or duplicate copy of the latest Report Card Academic/Character Reference (to be completed by your child’s current teacher).
Health Records: In the event that your child is accepted at St. Mel Elementary School, a completed Report of Health Form must be completed by a physician, signed by a parent and returned before the first day of school. This form includes a physical (dated within one year prior to the first day of kindergarten) and all L.A County mandatory immunizations must be completed before the first day of kindergarten. Please note a TB test is no longer required.
